LAB TEST INFORMATION
Laboratory blood tests are only required for athletes age 16+ who are participating in Full contact competition. If you are turning 16 this year and wish to continue competing after you birthday, have those lab tests ready to present to us! Please note lab work is only valid for 6 months, after which you will have to re-test.
Laboratory blood tests are only required for athletes age 16+ who are participating in Full contact competition.
Laboratory tests are required for all athletes age 16+ participating in Judged Full Contact competition, and are valid for 6 months following the date they are completed by a lab. It's important that the correct lab tests are completed early, or else they need to be redone and may not be ready in time for competition. The exact tests are listed on the MTO Semi-Annual Medical Declaration.

BACKGROUND SCREENING FOR COACHES & OFFICIALS
As part of the Ontario Sport Recognition Policy and Muaythai Ontario's Screening Policy, all Seconds (Coaches & Corners) and Officials are required to complete background screening. This background screening may be a Vulnerable Sector Verification (VSV), an Enhanced Police Information Check (E-PIC), or both. Please see our post on it here for more details.
Background screening documents are no longer required to be uploaded to the website. They must be shown at the competition - paper or digital. Without these documents, you will not be able to corner.

Please note: Background Screening is valid for 1 year. To complete an Enhanced Police Information Check, which can be done easily online for $25 at Sterling Talent Solutions.

Participation in any sport or physical activity carries with it the risk of injury. Concussions are an injury specific to the brain, and Ontario's Rowan's Law bring awareness to concussions and concussion safety. New regulation and legislation in Ontario requires that:
1. Athletes, parents of athletes under 18, coaches, and officials confirm every year that they have reviewed Ontario’s concussion awareness resources;
2. Members follow a removal-from-sport and return-to-sport protocol; and
3. Members follow a Concussion Code of Conduct that sets out rules of behaviour to support concussion prevention.
Muaythai Ontario takes concussion safety seriously, and requires that all members read through the concussion awareness resources and Concussion Code of Conduct as it applies to your age and role within the sport.

CONCUSSION CODE OF CONDUCT FOR ATHLETES

I will help prevent concussions by
- Wearing the proper equipment for my sport and wearing it correctly;
- Developing my skills and strength so that I can participate to the best of my ability;
- Respecting the rules of my sport or activity; and
- My commitment to fair play and respect for all (respecting Coaches, Officials, Physicians, and other Athletes).
I will care for my health and safety by taking concussions seriously, and I understand that
- A concussion is a brain injury that can have both short- and long-term effects;
- I don’t need to lose consciousness to receive a concussion;
- If I think I might have a concussion I should stop participating in further training, practice or competition immediately and inform a Coach, Official, Physician, Parent or other designated responsible person;
- If I suspect another athlete has sustained a concussion I should immediately inform a Coach, Official, Physician, Parent or other designated responsible person; and
- Continuing to participate in further training, practice or competition with a possible concussion increases my risk of more severe, longer lasting symptoms, and increases my risk of other injuries.
I will not hide concussion symptoms. I will speak up for myself and others
- I will not hide my symptoms. I will tell a Coach, Official, Physician, Parent or other designated responsible person if I experience any symptoms of concussion;
- If someone else tells me about concussion symptoms, or I see signs they might have a concussion, I will tell a Coach, Official, Physician, Parent or other designated responsible person so they can help;
- I understand that if I have a suspected concussion, I will be removed from sport and that I will not be able to return to training, practice or competition until I undergo a medical assessment by a medical doctor or nurse practitioner and have been medically cleared to return to training, practice or competition;
- If I am diagnosed with a concussion, I understand that I must inform all my Coaches and Teachers across all the sports and Sport Organizations I participate in about my injury, and this will help them support me while I recover; and
- If I am diagnosed with a concussion, I understand that I must inform Muaythai Ontario through its Injury Reporting form at muaythaiontario.org/injuries.

- I understand I will have to follow my sport organization’s Return-to-Sport Protocol;
- I understand I will have to be medically cleared by a medical doctor or nurse practitioner before returning to training, practice or competition; and

CONCUSSION CODE OF CONDUCT FOR COACHES & OFFICIALS

I will help prevent concussions through my
- Efforts to ensure Athletes wear the proper equipment and wear it correctly;
- Efforts to help Athletes develop their skills and strength so they can participate to the best of their abilities;
- Respect for the rules of my sport and ensure that my athletes do, too; and
- My commitment to fair play and respect for all (respecting coaches, officials, and all participants and ensuring my athletes respect others and play fair).
I will care for the health and safety of all participants by taking concussions seriously. I understand that
- A concussion is a brain injury that can have both short- and long-term effects;
- A person doesn’t need to lose consciousness to receive a concussion;
- An athlete with a suspected concussion should stop participating in training, practice or competition immediately;
- Continuing to participate in further training, practice or competition with a suspected concussion increases a person’s risk of more severe, longer lasting symptoms, and increases their risk of other injuries or even death;
- I have a commitment to concussion recognition and reporting, including self-reporting of possible concussion and reporting to a designated person when someone suspects that another individual may have sustained a concussion; and
- If an athlete under my care sustains a concussion outside of Muaythai Ontario sanctioned competition I will ensure that they inform Muaythai Ontario through the Injury Reporting form at muaythaiontario.org/injuries or I will report it on their behalf.
I will create an environment where participants feel safe and comfortable speaking up. I will
- Encourage athletes not to hide their symptoms, but to tell me, a Coach, an Official, a Parent or other designated responsible person they trust if they experience any symptoms of concussion after an impact;
- Lead by example. I will tell a Coach, Official, Parent and seek medical attention by a Physician or Nurse Practitioner if I am experiencing any concussion symptoms;
- Understand and respect that any athlete with a suspected concussion must be removed from sport and not permitted to return until they undergo a medical assessment by a Physician or Nurse practitioner and have been medically cleared to return to training, practice or competition; and
- For Coaches only: Commit to providing opportunities before and after each training, practice and competition to enable athletes to discuss potential issues related to concussions.
I will support all participants to take the time they need to recover
- I understand my commitment to supporting the return-to-sport process;
- I understand the athletes will have to be cleared by a Physician or Nurse Practitioner before returning to sport; and
- I will respect Coaches, Officials, Parents, Physicians and Nurse Practitioners and any decisions made with regards to the health and safety of my athletes.

I, the undersigned, acknowledge that my membership requires that I understand and adhere to the rules, regulations and policies of Muaythai Ontario (MTO). Failure to do so at all times will result in my suspension from events and/or the suspension or the cancellation of my membership.

I give permission for my name and any image of myself, taken by officers/staff/volunteers of Muaythai Ontario, to be used for promotions and publications of the organization in accordance with Muaythai Ontario’s Privacy Policy. I further give MTO permission to share the same with it's national and international affiliates (e.g. Muaythai Canada, International Federation of Muaythai Amateur, Government Ministries, Olympic Committee).

I further understand that by signing this document I agree that throughout the course of my membership with Muaythai Ontario that:

1. Allocation of Risk on Athlete I understand there are significant risks involved in all aspects of Muaythai and full contact fighting in general that may result in serious bodily or psychological injury, permanent disability, paralysis, or death. I also understand that there is a risk of incurring some injuries that may not become apparent until some time after the fight, which may also result in permanent disability or death, and I:
1. a. Willingly and voluntarily expose myself to the risks above;
2. b. Accept full responsibility for any injury or death, whether caused by me or caused by the actions or inactions of other competitors, event host, Muaythai Ontario (“MTO”) officials, staff and volunteers;
3. c. Assume all responsibility for any losses, costs, and damages that may be incurred as a result of my participation at any event.
2. If at any point before or during my bout I believe conditions to be unsafe, I will immediately notify the referee, or any other official, and if necessary discontinue further participation in the activity.
3. Medical I warrant that any information I gave to my physician for the purposes of the Annual Medical Declaration, and any information given by me during the Pre-Competition Medical is true. I realize that by providing false information I increase the risk of serious injury or death occurring to me and/or other participants. I understand that if I provide false information, I may be removed from the competition and prohibited from participating in any MTO events. I further accept that I have a continuing obligation to update Muaythai Ontario regarding any changes in my medical history.
4. Drugs I promise not to consume and not to be under the influence of any drugs or prohibited substances outlined in sections 13 and 14 of the MTO Medical Handbook. I understand that if such substances or traces of such of such substances are found in my system, it will result in my disqualification from the competition.

5. In consideration of being allowed to participate at any event, I hereby fully release MTO, their agents, employees, affiliates, and volunteers from any and all liability and legal action that may arise, directly or indirectly, out of my participation at any event, as well as liability incurred as a result of negligent acts or omissions of the above parties.
6. I understand that by placing my initials in this section and signing the bottom of this document, I agree to indemnify MTO and the above parties from any liability for injury or death caused to any other person, as well as damage to the property, however caused by me.
7. I understand that this release shall be binding upon me, my heirs, executors, administrators and successors.

(Additional Waiver Terms if acting as a Second)

8. Duty to Athlete I understand that the athlete I am cornering will be exposed to risks that are inherent in Muaythai and full contact fighting in general. Such risks may result in serious injury, disability, paralysis or death. I also understand that as a second I have a duty to ensure the Athlete is safe at all times. Therefore, I agree to do the following:
1. a. I will attend the pre-competition rules meeting arranged by the Chief Official where the MTO rules will be conveyed to me.
2. b. Before the round begins, I will ensure that all the equipment such as buckets, seats, etc. is removed from the ring.
3. c. I will not intentionally or negligently provide my athlete with bad advice, assistance or encouragement that I know or reasonably ought to know could cause, or increase the risk for injury or death.
4. d. I will not encourage or incite the spectators by words or signs to advise or encourage/discourage an athlete during the competition.
5. e. Coach Only I will be in possession of a towel for the athlete. During the match, if I believe my Athlete is in danger or any other precarious condition, I will throw in the towel to give up for the Athlete and stop the competition.
9. Drugs I will not administer, or encourage the athlete to consume any drug or prohibited substance outlined in the World Anti-Doping Association (WADA) guidelines, which were provided to me either before or during my signing this agreement.
10. Duty to Other Competitors I will ensure that my Athlete is wearing proper attire, equipment, free from any objects or other prohibited articles of dress that may expose the Athlete or others to a risk of injury, prior to entering the ring.
11. Penalty I understand that if I violate any of the rules outlined in the MTO Provincial Rules and Regulations, the rules conveyed to me at the Chief Official’s meeting, the MTO Code of Conduct, or any clause of this document, MTO reserves the right to:
1. a. Disqualify and/or remove me from the competition. I understand that if ordered by the referee to remove myself, I must vacate the competition area for the remainder of the session;
2. b. Prohibit a disqualified second from acting as a second for the remainder of a particular session, or the entire competition/tournament where the offence was committed;
3. c. Caution, penalize or disqualify the Athlete for whom the second is acting.
12. I hereby promise to act in the best interest of the Athlete and to obey all the rules and obligations imposed on me by MTO.
13. Severability and Entire Agreement This waiver is complete, and there were no representations made to me that contradict anything stated in this document. If any portion of this waiver is held invalid, the remainder of the waiver shall remain in force.
