This form is for athletes who have already completed their Competitive Membership with Muaythai Ontario, but need to submit or update their medical information. Annual Medical Declarations and blood work are only valid for 12 months from the date shown on the completed form.
Effective immediately membership information will no longer be accepted within 48 hours of an event.
Membership information submitted to Muaythai Ontario takes 7 days to process, review, and verify. Athletes, coaches, and officials must be diligent in submitting all membership information (including medical, lab tests, and vulnerable sector checks) at least 7 days prior to an event in which they intend to participate. Members will have access to a mobile passport to present membership information at MTO sanctioned events.
Individuals submitting information between 2 and 6 days prior to an event will be able to pay a fee of $25 for rush processing
All athletes participating in full contact competition must have a completed Annual Medical Declaration that has been completed no more than 12 months prior to the competition date.
All athletes age 16 years and older participating in full contact competition must have HIV, Hepatitis B Surface Antigen, and Hepatitis C Antibody screening blood tests that have been completed no more than 12 months prior to the competition date.
