The request for reinstatement in to amateur competition is for athletes who have participated in professional combat sports competition, Muaythai competition in Thailand, or are presently licensed with an athletic commission in North America.
Athletes who reinstate as amateurs are automatically considered to be A Class, however a request can be made to enter B Class or C Class based on a review of the athlete's abilities, experience, and past performance. To be eligible for placement in a less experienced class, the athlete must still fall within the experience requirements (0-3 contests for C Class, 4-9 contests for B Class) and may be required to provide supporting evidence of their professional experience.
After completion, the form will be reviewed by a technical committee for approval. Athletes with professional experience in North America are subject to additional review and approval by the Ontario Athletic Commissioner.

I certify that the above information is complete and true, and I am authorized to make this request on behalf of the athlete. I understand that the request to reinstate in amateur competition under Muaythai Ontario is dependent upon review by a technical committee. Should the request be approved and the athlete returns to professional competition at a later date, any future requests for reinstatement will result in automatic placement in A class if further reinstatement is approved.
Making false statements may jeopardize your ability to reinstate and can result in the suspension or cancellation of a professional license and impact your ability to license at a future date. Further, false statements will be subject to disciplinary action per Muaythai Ontario's Code of Conduct.
